A certificate is required in the following cases:

  • when registering a child for kindergarten;

Having a vaccination certificate is convenient and economical. The extract allows you to receive some documents free of charge (medical certificate for a sanatorium, medical card for a school or kindergarten).

What does it look like?

Information about the vaccination performed is included in a special standard certificate. The form of such a document is approved by the Ministry of Health of Russia (order No. 220 of September 17, 1993). The certificate form and a sample for filling it out can be downloaded on the Internet or taken from a local clinic.

In the upper left corner the name of the institution is indicated - the Ministry of Health of the Russian Federation. In the right corner are codes according to OKUD, institutions according to OKPO.

The title of the document is located in the center. Below on the left is the date of registration. To the right it is indicated for which institution the certificate is issued. Below is the surname, patronymic, name of the person to whom the document is issued, address and date of birth of the person.

The table lists all the information about the vaccinations given:

  • date of immunization;
  • name of the drug;
  • dosage;
  • vaccine series;
  • vaccination site;
  • reaction.

It is important that the document is filled out correctly, without blots or strikethroughs. Otherwise it will be considered invalid.

Filling out the vaccination report

The report is intended to convey information about vaccinated individuals. It consists of two tables. The first contains basic information about the employer:

  • its full and short name;
  • TIN and OGRN (OGRNIP);
  • legal address of the organization or registration address of the entrepreneur;
  • main and additional activity codes;
  • data on the total number of employees.

Information about each employee who has received a preventive vaccination is entered into the second table. It contains the following columns:

  • SNILS;
  • compulsory medical insurance policy number, if any;
  • series and number of passport or other identity card;
  • contact phone number (mobile).

Where can I get a vaccination certificate for kindergarten and school?

Information about immunization is reflected in the vaccination card and certificate.
These documents have equal force and significance. They are stored at the local clinic. There are duplicates in institutions where such documents were previously submitted, and at home. A person does not always keep a vaccination record.

Then the question arises, where can you get it? If parents are going to register their child for kindergarten, they will be required to provide a vaccination certificate.

An extract can be obtained from a pediatrician at the clinic at your place of registration . This medical institution maintains a preschool and school medical record card, which contains a section on immunoprophylaxis. The pediatrician also issues a certificate about the epidemiological environment.

Where can I get a vaccination certificate for an adult for work?


Photo 4When applying for a job, they usually require a statement of vaccinations. To obtain a document, an adult must contact the clinic at the place of residence or another institution that has such information (student clinic, previous place of work).

If you still have a copy of the certificate at home, then you can provide it. But first, this document must be certified by the doctor’s signature and the stamp of the medical institution.

Another way is to contact a private company that processes medical documents (vaccination cards, certificates, insurance).

This approach allows for immunoprophylaxis to be carried out in a timely manner. Documents on immunoprophylaxis are stored for 5 years. The validity period of the immunization certificate is limited to the period of presence of antibodies in the body.

For example, a flu vaccination document is valid for 12 months. The official form for vaccinations begins to be filled out at the first vaccination from infancy.

Medical certificates required for entry into the camp

To successfully pass the medical examination upon departure to the camp, you must provide the following documents:

  • Certificate in form 079/у

According to Order No. 834 of December 15, 2014, the validity of this certificate is no more than six months from the date of issue.

Certificates of the established form from an educational institution or medical institution (polyclinic, medical center, medical center, etc.) with a triangular, rectangular seal (address of the institution), as well as a round seal of a pediatrician, if available, are accepted).

Please note that the certificate in paragraph 7 requires information about the p test performed. Mantu valid for no more than a year before the date of departure to the camp.

  • Certificate of epidemiological environment (contacts), with information that no infectious diseases have been registered within 21 days. The certificate must be issued no earlier than 3 days before departure to the camp.
  • If there is a swimming pool in the camp, you must provide a standard certificate of admission to visit the swimming pool from a medical institution based on SanPiN 2.1.2.1188-03 “Swimming pools”. The certificate is valid for 3 months from the date of issue.
  • A trip to the camp.
  • Original and copy of birth certificate for children under 14 years old , original and copy of passport (first page spread with photo and registration page for children over 14 years old .
  • A copy of the compulsory health insurance policy.
  • A certificate indicating the results of an analysis for helminth eggs and enterobiasis and a copy thereof (for children under 11 years of age). Please note: if you are preparing a certificate for the pool, then the results of the analysis for helminth eggs and enterobiasis are indicated in it, in which case a separate certificate is not required.
  • In case of chronic diseases, medications must be accompanied by a certificate from the attending physician with the seal of the medical institution.

During a medical examination, doctors check not only the presence of all the necessary certificates and notes on the vaccinations given, but also examine the child to exclude the possibility of the presence of diseases based on external signs (head lice, fungal infections, etc.).

Please note that the examination is carried out by current qualified medical personnel of Moscow city clinics (senior nurse, pediatrician, head of the pediatric department).

Here the certificate is issued promptly: within an hour. Many companies offer to obtain a fictitious document. This option is optimal for those who do not want to be vaccinated.

A certificate of vaccination against measles, flu, hepatitis or other diseases in private organizations is issued on average for 800-1000 rubles. If necessary, company employees can make a medical exemption from vaccination. This service costs a little more – 1000-1200 rubles.

Vaccination certificate form 156/u-93 can be purchased for 2,300 rubles. A certificate of Mantoux reaction costs an average of 800 rubles. A preventive vaccination card in form 063/u costs about 1,000 rubles. You can purchase a vaccination certificate with delivery.

It is not recommended to make fictitious certificates. Lack of vaccinations increases the risk of contracting a dangerous pathology. If vaccination is contraindicated, it is better to make a medical withdrawal. With such a document they are also taken to educational institutions and holiday homes.

Medical certificate form 063/U (Vaccination sheet) consists of the following parts:

  • the name of the medical institution in the upper left corner;
  • Name;
  • date of registration of the patient;

Normative base

Each state has developed its own vaccination calendar depending on what infectious diseases are relevant to the country. This is a form of primary medical documentation regulated by order No. 1030 of October 4, 1980 of the USSR Ministry of Health (as amended on December 31 2002).
