What it is

A vaccination card is an official document containing summary information about a person’s vaccinations and revaccinations. It is drawn up and stored in the outpatient clinic where the patient is observed.

The preventive vaccination card (that is, form 063/u) contains not only a list of all vaccines administered, but also the following information:

  • date of vaccination;
  • serial number of the drug that was administered;
  • vaccine dosage;
  • method of vaccination, place of injection of the drug. So, for young patients, some compounds are injected into the thigh, and for older children - into the arm;
  • local reaction. For example, when performing the Mantoux test, the diameter of the papule is indicated.

The document itself (also called a vaccination record sheet) contains information about all vaccinations given to the child since birth, including the date of vaccination, dose, number and series of injections, expiration date, and the body's reaction to the vaccine.

This is the most important information about the child. In addition, this card indicates the child's full name, date of birth, permanent residence address and home telephone number.

This certificate is issued primarily for enrolling a child in kindergarten. If such a vaccination card 063/ has not yet been issued, this can be done in some kindergartens.

The form itself may differ slightly from the standard form issued by clinics in schools and kindergartens. There is no need to worry about this or not to trust such a certificate.

In the case of children, however, the vaccination card not only shows the number of vaccinations given to the child, but also guarantees the protection of the child, who is exposed to many risks in a large group - which is why Form 063 for kindergarten is so carefully designed.
